These Official Rules (the “Rules”) govern the TruDiagnostic® Black Friday Giveaway (the “Sweepstakes”). By entering, you agree to be bound by these Rules and Sponsor’s decisions, which are final and binding.
1. ELIGIBILITY
The “TruDiagnostic Black Friday Giveaway” (“Sweepstakes”) is open to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older at the time of entry. Employees, contractors, directors, officers and agents of TruDiagnostic, Inc. (“Sponsor”), its affiliates, subsidiaries, advertising and promotion agencies, and members of their immediate families (spouse, parent, child or sibling, whether biological, adopted, step or in-law) or households (whether related or not) are not eligible to enter or win. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations and is void where prohibited. Participation constitutes entrant’s full and unconditional agreement to these Official Rules and Sponsor’s decisions which are final and binding in all matters related to this Sweepstakes. Winning a prize is contingent upon fulfilling all requirements as set forth herein.
2. SPONSOR
TruDiagnostic, Inc., 881 Corporate Dr, Lexington, KY 40503 (“Sponsor”).
3. SWEEPSTAKES PERIOD
The Sweepstakes begins at 12:00:00 a.m. Eastern Time (ET) on November 28, 2025 and ends at 11:59:59 p.m. ET on November 28, 2025 (the “Sweepstakes Period”). Sponsor’s computer web server is the official time-keeping device for the Sweepstakes.
4. HOW TO ENTER
There are two (2) ways to enter:
(a) Purchase Entry (Automatic Entry)
During the Sweepstakes Period, visit trudiagnostic.com (“Website”) and complete a purchase of any of the following tests: TruAge®, TruHealth™, or the TruAge + TruHealth Combination Kit (each a “Qualifying Product”). Eligible entrants who make a Qualifying Purchase during the Sweepstakes Period will automatically receive one (1) entry into the Sweepstakes. Only purchases made directly through trudiagnostic.com qualify. Canceled, refunded, or fraudulent orders are void.
(b) Free Digital Entry (No Purchase Necessary)
During the Sweepstakes Period, visit trudiagnostic.com/blackfriday (“Free Entry Website”) and follow the online instructions to complete and submit the official entry form (“Free Entry”). You must provide your full name, valid email address, and state of residence. Submissions generated by script, macro, or other automated means are prohibited and will be void.
For all entries: Limit one (1) entry per person during the Sweepstakes Period, regardless of method of entry. Multiple entries will be void. Entries that are incomplete or do not comply with these Official Rules are void.
5. SWEEPSTAKES DRAWING
One (1) potential winner will be selected in a random drawing conducted by Sponsor on or about December 3, 2025, from among all eligible entries received during the Sweepstakes Period. Odds of winning depend on the total number of eligible entries received. Sponsor’s decisions are final in all matters related to this Sweepstakes.
6. WINNER NOTIFICATION AND VERIFICATION
Potential winner will be notified by email or phone within approximately five (5) days of the drawing. The potential winner must respond within forty-eight (48) hours of notification and may be required to complete, sign, and return an Affidavit of Eligibility and Liability/Publicity Release within forty-eight (48) hours of receipt. If the potential winner fails to respond or comply, is deemed ineligible, or declines the prize, an alternate winner may be selected, time permitting.
7. PRIZE DETAILS
One (1) prize will be awarded. The winner will receive a one-year TruAge + TruHealth with Combination Kit Membership (Approximate Retail Value “ARV”: $1,999). The membership entitles the winner to standard features and benefits of the TruAge + TruHealth Combination Kit plan as described on the Sponsor’s Website at the time of fulfillment. Prize is non-transferable and has no cash value. Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable.
8. PRIZE REDEMPTION
Winner will receive electronic redemption instructions via email within approximately two (2) weeks after verification. Membership must be activated by January 31, 2026, or the prize will be forfeited. The one-year term begins upon activation. A TruDiagnostic account is required to redeem the membership.
9. TAXES
All federal, state, and local taxes, and any other costs associated with acceptance and use of the prize not specified herein as being provided, are the sole responsibility of the winner. Sponsor will issue tax documentation if required by law.
10. LIMITATIONS AND CONDITIONS
Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes if fraud, technical failures, or any other factor impairs its integrity or proper functioning. Proof of entry does not constitute proof of receipt. Sponsor is not responsible for lost, late, incomplete, misdirected, or undeliverable entries.
11. PUBLICITY
Except where prohibited, acceptance of the prize constitutes permission for Sponsor to use the winner’s name, city/state, likeness, and biographical information for advertising and promotional purposes without additional compensation.
12. RELEASE
By entering, participants agree to release and hold harmless Sponsor, its affiliates, subsidiaries, and advertising and promotion agencies, and their respective officers, directors, employees, and agents, from any and all liability, loss, or damage arising out of participation in the Sweepstakes or acceptance, use, or misuse of any prize.
NOTICE TO NEW JERSEY RESIDENTS: The limitations and exclusions in this Release may not apply to you to the extent damages are the result of Sponsor’s negligent, fraudulent, or reckless acts or intentional misconduct.
13. HEALTH & REGULATORY DISCLOSURE
TruDiagnostic’s tests and memberships are not medical care or insurance and are not intended to diagnose, treat, cure, or prevent disease. Testing may require physician authorization and may be unavailable in certain states. Always consult a qualified healthcare provider for medical advice.
14. GOVERNING LAW AND DISPUTE RESOLUTION
All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules, or the rights and obligations of entrant and Sponsor, shall be governed by the laws of the State of Kentucky, without regard to its conflicts of law principles. Except where prohibited, any dispute arising from this Sweepstakes shall be resolved by binding arbitration under the Federal Arbitration Act in Lexington, Kentucky, pursuant to the AAA Consumer Arbitration Rules, with no class, collective, or representative actions permitted.
15. PRIVACY
Information collected in connection with this Sweepstakes will be used in accordance with Sponsor’s Privacy Policy located at https://www.trudiagnostic.com/privacy-policy.
16. WINNER’S LIST
For the name of the winner (first initial, last name, city, and state), send a self-addressed stamped envelope to: TruDiagnostic Black Friday Giveaway Winner’s List Request, TruDiagnostic, Inc., 881 Corporate Dr, Lexington, KY 40503, to be received by January 31, 2026.
